Болестта на фон Вилебранд в детската оториноларингологична практика – серия от клинични случаи и практически препоръки

Резюме

Болестта на фон Вилебранд (БФВ) е вродено нарушение на хемостазата, но в детска възраст остава често неразпознатa. Една от водещите изяви е рецидивиращият епистаксис.

Цел. Да представим серия от клинични случаи при деца с първа изява в оториноларингологичната практика и да предложим кратък въпросник за насочена анамнеза.

Материал и методи. Ретроспективна серия от три клинични случая: момче с рецидивиращ епистаксис; семейство с двама братя, диагностицирани съответно предоперативно и след рецидивиращ епистаксис; и момиче на 3 години с тежко кървене след аденоидектомия.

Резултати. И в трите случая рутинните коагулационни показатели се оказаха недостатъчни – решаваща беше насочената хеморагична и фамилна анамнеза.

Заключение. Анамнезата често превъзхожда рутинния коагулационен скрининг при отсяване за специфична диагностика, в съгласие с международните препоръки. Предлагаме кратък въпросник и подчертаваме нуждата от обучение на лекарите и от информираност на пациентите.
