Tравматичен панкреатит в детска възраст

Резюме

Травматичният панкреатит в детска възраст е рядко, но потенциално животозастрашаващо състояние, най-често резултат от тъпа коремна травма с директен удар в епигастралната област.
Клиничната изява често е неспецифична и може да варира от леки коремни болки до остър хирургичен корем и шок, което затруднява ранната диагноза. Лабораторните показатели и образните изследвания имат ключова роля за своевременното разпознаване и оценка на тежестта на панкреасното увреждане.
Авторите представят клиничен случай на 9-годишно момче, прието по спешност след падане с велосипед с директен удар на кормилото в епигастриума. Пациентът постъпва в тежко общо състояние с клиника на остър хирургичен корем и шок. Лабораторните изследвания установяват изразен възпалителен процес и значително повишени стойности на панкреасните ензими.

Образните и интраоперативните находки потвърждават панкреасно засягане, хемоперитонеум и ретроперитонеални хематоми. Провежда се спешно оперативно лечение, последвано от продължително консервативно поведение и активно проследяване.

В хода на заболяването се установяват екзокринна панкреасна недостатъчност и асимптомна посттравматична панкреасна псевдокиста с малки размери, подлежаща на проследяване. Случаят подчертава значението на ранната диагноза, мултидисциплинарния подход и дългосрочното терапевтично поведение и проследяване при деца с травматичен панкреатит.
